We understand that there are a variety of services needed for elderly or aging parents or patients and we are committed to providing an individualized plan to ensure that the needs and values of each individual are prioritized. We start with a functional assessment including a physical, mental, and environmental assessment to determine which areas need attention first. Once we know what is needed, we can provide a variety of services, including:
- regular home visits
- advocacy at doctor and mental health appointments and follow through with treatment plans and medications
- providing respite care (and holding power of attorney when family members are away and need assistance)
- assistance in senior living and housing arrangements, or relocation services
- offering support for mental health disorders and diseases, including Alzheimer’s, bipolar disorder, PTSD, depression, schizophrenia, and other disabilities
- guardianship to allow for decision making when family members are unable or unavailable
- resources for family members
- other support services that are needed depending on your client or family member’s needs
