- Our fully background checked care managers work with clients, families, conservators, attorneys, financial institutions, and others to meet the unique needs of the client.
- Gather psychosocial histories, as well as medical and personal information to create an assessment that will determine the appropriate care and services for the client.
- Advocate for the client’s health by working with their doctors, nurses, dentists and other medical professionals to ensure the best outcomes.
- Transport or arrange transportation for the client, as necessary.
- Attend medical appointments. Keep the client, family and care facility updated on changes of condition and medications.
- Monitor mental and physical condition of client and schedule medical appointment as needed.
- Advocate for client while in the emergency room and during hospital stays.
- Work with medical discharge planners and social workers to assure a smooth transition, with trusted housekeeping, shopping, and support services in place.
- Ensure that the client’s medical choices are carried out as planned.
- Conduct regular home visits to assess the client’s health and living conditions.
- Schedule outings for shopping, entertainment and social activities.
- Arrange and coordinate moving clients to or from private homes, retirement communities, assisted living communities or nursing facilities.
- Serve as a representative for a Health Care Durable Power of Attorney.
- Act as a liaison to families who are on vacation, travelling, or live out of the area.
- Provide crisis intervention, counseling, and support to the client and/or family.
